    The CPDM team includes professionals from: 1. Nursing; 2. Occupational therapy; 3. Physiotherapy; 4. Psychology; 5. Psychometry; 6. Therapeutic recreation; 7. Social work; 8. Stenography/secretarial; 9. Consulting medical specialties; and 10. Consulting dietetics.

    Most CPDM referrals come from physicians or other health professionals. We also accept self-referrals. Referrals are placed on a wait-list and are seen in the order of the date of referral.
