Supreme Calamitas does not spawn on her own and requires the player to summon her with the Altar of the Accursed, which uses Ashes of Calamity or the Ceremonial Urn; upon using said items at the Altar, the screen will darken, shake, and become tinted red before Supreme Calamitas herself appears above the Altar. The Ceremonial Urn is reusable, so Supreme Calamitas may be summoned any number of times. When Supreme Calamitas is fully summoned, she will laugh devilishly instead of using the regular boss roar. Supreme Calamitas can be fought in any biome at any time.

If the Brimstone Witch NPC is currently alive in the world, she will disappear when Supreme Calamitas is summoned.

Upon spawning, Supreme Calamitas will create a 159x159 (inner available area is 157x157) square arena made of mostly unbreakable Arena Blocks. Leaving it will cause Supreme Calamitas to enrage, significantly increasing the velocity of all of her attacks and giving her 99% irreducible damage reduction.

Supreme Calamitas has several powerful attacks that she will switch between in a consistent cycle. She always remains either directly above or to the side of the player, only breaking position during Bullet Hell phases or during her devastating dash attacks.

•Supreme Calamitas takes decreased damage from some items and projectiles. She takes:

    •90% damage from Onyxia.

•50% damage from the Draconid minion summoned by Yharon's Kindle Staff.

    •Supreme Calamitas' brothers take:

•90% damage from Phoenix Flame Barrage.

    •Supreme Soul Seekers take:

•The Brimstone Witch will begin to arrive if provided suitable housing, allowing the player to utilize the Enchantment system.

•In subsequent rematches, she will appear unhooded and will use a different set of dialogue lines.

•Each Bullet Hell segment lasts for an exact 15 seconds.

•During Bullet Hell segments, brothers phase, or when at 1% health, Supreme Calamitas cannot deal contact damage to the player.

•Hearts (such as from Heart Statues) have halved healing amounts while Supreme Calamitas is alive, making it harder to recover health.

•The Adrenaline Meter in Revengeance and Death Mode will fill up at a slower rate. It will take 45 seconds to fill up during this fight rather than the typical 30 seconds.

•Supreme Calamitas and her brothers have decreased acceleration if the player is wielding a True melee weapon.

•Supreme Calamitas' attacks seem to ignore invincibility frames, this can be proven if Joost Mod is installed and the player holds a cheat item from the mod (e.g. The Ultimate Fist or Tome of Greater Manipulation) which provides infinite i-frames, therefore proving that her attacks ignore said i-frames.

•Making an arena can potentially increase mobility, which is useful for dodging Supreme Calamitas' fast attacks.

•Supreme Calamitas' dash is fast and can sometimes deal fatal damage, so it's recommended to attack with weapons that have decent range and/or homing ability which will make it easier to dodge her dashes.

•Don't bother trying to attack Supreme Calamitas during the Bullet Hell phases or when the Sepulcher and its Brimstone Hearts are still alive. She will be immune to damage during them.

•Try to master the first Bullet Hell phase. The projectiles are far more damaging at the beginning and they deal less damage in the further phases, so it's better to take as little damage as possible in the first attack.

•During the first Bullet Hell phase, do not panic and move about too much as the blasts in this phase are extremely deadly and occasionally spawn in groups. Getting hit during this phase could potentially lead to the player's early demise.

•During Bullet Hell phases, it's good in some cases to stick to a wall when the projectiles are only moving from one side, as it may be easier to dodge them. Be sure to watch out for the Fireblasts and Gigablasts that home in on the player.

•The song heard during the battle against Supreme Calamitas is Stained, Brutal Calamity, which was composed by the artist DM DOKURO. This song contains full lyrics sung by a choir as well as DM DOKURO himself. Different sections of the song play as the fight progresses (at 100% / 50% / 30% / 1% boss health), each section representing a different perspective of the fight.

•The version that plays in-game is slightly modified in order for the track to loop properly:

•"Grief" plays from the start of the battle to the third Bullet Hell.

•"Lament" plays from the third Bullet Hell until the fourth.

•"Epiphany" plays from the fourth Bullet Hell until she stops attacking.

•"Acceptance" plays when she stops attacking.

•"Do you enjoy going through hell?" (When summoned)

•"You should have just died..." (When the fight begins after the first bullet hell)

•"It wasn't too long ago you barely managed to defeat my doppelganger. Quite the failure, wasn't it?" (During the second bullet hell)

•"You've harnessed great power, but you wield it for no one but yourself." (During the third bullet hell)

•"Would you like to meet my family? Horrible, isn't it?" (When summoning Supreme Cataclysm and Supreme Catastrophe)

•"You will suffer greatly." (When transitioning into her second phase)
