I think most people who could easily afford it would continue seeing a top psychiatrist, because some of them actually do talk therapy, plus they understand the medical aspect behind it. Melfi was one of the psychiatrists who also does talk therapy.
Tony used the therapy session to express his lack of remorse (without revealing what he actually did). He never resolved his bigotry, misogyny as highlighted by his parting words to Dr. Melfi: 'I blame this on female menopausal symptoms'.
