  3. For the record, where I'm from (Ontario, Canada), we have to pass a literacy test in grade 10 in order to graduate from high school. If you don't pass, you have to do it again. If you still don't pass, you get placed in a special class to help become literate.

  4. Best, fastest way to research this for your particular novel would actually be to decide what state the person lives in, and go from there. Contact some state department of social services and just say you're a novelist researching this. Shouldn't be that hard to figure out who would know.
