Health coaching effectively improves hypertension self-care activities and the control of blood pressure (BP) in hypertensive patients. Studies on the effects of health coaching on patients in primary care with uncontrolled hypertension in developing countries are limited.
This is thought to be due to a combination of poor patient education, poor medication adherence, and "clinical inertia." The present trial evaluates an intervention consisting of health coaching, home blood pressure monitoring, and home medication titration as a method to address these three causes of poor hypertension control. As shown in Table 1, participants selected a variety of LSM plans for control of BP and improvement of health. Project findings support health coaching as a promising model for sustaining patient engagement in selected LSM plans and medication adherence for improved BP outcomes.
