Individuals with uncontrolled hypertension need to perform ongoing hypertension self-care to improve their blood pressure (BP) control, prevent hypertension-mediated organ damage, improve quality of life, and decrease the all-cause mortality rate.
As shown in Table 1, participants selected a variety of LSM plans for control of BP and improvement of health. Project findings support health coaching as a promising model for sustaining patient engagement in selected LSM plans and medication adherence for improved BP outcomes.
