For example, Meg isn’t supposed to understand Stewie, but she understands him in season 10, episode 20, “Leggo My Meg-O,” in which he saves her from European kidnappers.
So there's actually a list of who can understand him, it includes: Brian obviously. Peter Griffin ( who explicitly said, "Thank you Stewie, who I can understand.") The whole show is Stewie's interpretation of the world. How an infant sees the world and tries to understand it from his limited knowledge. That's why each episode seems so exaggerated, so extreme. Stewie is interpreting his family's actions and filling in the blanks due to his limited vocabulary and understanding of the world.
There was a good joke in one episode where Stewie and Brian were debating if a side character (I forget who) could understand Stewie, with them claiming that if that character was too close to being a main cast member then they can't understand him.
