- Niacinamide. According to Dr. Green, niacinamide, a powerful antioxidant derived from vitamin B3, can be effective for acne-prone skin. “This ingredient fights acne by reducing inflammation and dark spots,” she says.
- Azelaic acid. Dr. Green says azelaic acid works to unclog pores, lightens dark spots, soothes inflammation and reduces redness in your skin—it’s the perfect weapon for fighting dreaded cystic acne.
- Adapalene. While Dr. Sperling says cystic acne patients usually need a more aggressive treatment, she says a product like Differin Gel can help. Differin contains adapalene, a topical retinoid, which targets acne deep beneath the skin’s surface while improving texture.
- Retinol. Retinol, with its high-cell-turnover powers, is ideal for treating acne-prone skin as well as improving the appearance of scars caused by severe acne.

- Get a Cortisone Shot. If you want to go straight to the source, ask your dermatologist to give you a shot of cortisone/diluted steroid. It's injected straight into the cyst and quickly reduces inflammation.
- Make an At-Home Aspirin Mask. For cystic acne (those pesky, red, painful, under-the-surface bumps), both Jennifer Herrmann, MD, and Joshua Zeichner, MD, recommend a simple at-home mask consisting of crushed aspirin and water.
- Apply a Spot Treatment. Apply a triple threat of acne-fighting ingredients. "Benzoyl peroxide lowers levels of acne-causing bacteria, hydrocortisone cream reduces inflammation, and salicylic acid removes excess oil to dry out a pimple," says Dr. Zeichner.
- Mix Together a Vinegar Cleanser. "Diluted white vinegar is an amazingly broad antibacterial that can help kill the acne bacteria," explains Dr. Herrmann.
