General. All school boards are required to review documentation to determine a student’s eligibility to attend a publicly funded school in Ontario either with the payment of a fee or as a provincially funded student. School boards should be aware of their obligations and responsibilities under the Education Act when carrying out their review.
School boards should use the revised Student Information and Eligibility Attestation Form (formerly Pupil Eligibility Attestation Form) or equivalent to record a student’s information and attest to verifying relevant documentation to support a pupil’s residency and eligibility.
