In this article, we will explore the essential steps to safeguard patient and staff safety and prevent incidents in healthcare. Following these steps can create a safety culture, enhance communication, improve training and education, implement effective procedures, and continuously monitor and evaluate patient safety efforts.
Therefore, this chapter discusses organizational and systems strategies that can help to (1) establish a culture of safety, (2) improve handover processes, (3) use adverse event and error-reporting systems for resident learning, and (4) develop a team culture to improve communication and task performance among residents.
This paper puts forward an argument for themed reviews of patient safety incidents and provides an illustrative template for theming incidents using a human factors classification tool.
