We are actually not given a clear picture of exactly how she died. There are two separate accounts; one by a drunken Fleabag to the cabbie at the end of S1E1 (ie the pilot) and then Boo's (reported) speech in S1E6. "Yeah. She's dead now. She accidently killed herself.
It seems that by saying it was accidentally Fleabag is making Boo seem vindictive, manipulative and socipathetic. Boo would have been fine if only fate hadn't intervene. While Boo intentally step in front of a bike to end her life was directly cause by Fleabag's betrayal. In the first explanation Fleabag doesn't own up to her action.

Boo is a recurring character in the first and second series of the comedy drama Fleabag. She was Fleabag's best friend, and co-owner of the cafe that the two of them ran. She is played by Jenny Rainsford.
Boo accidentally killed herself after learning that her boyfriend had cheated on her. She intended to injure herself by stepping out into a bike lane, but the resulting crash caused the death of three people, including Boo.
