What I think it (and the whole series) means is that sexual abuse is a disease that is passed from one victim to the next. Notice that Killer Bob is the representation of a guy who sexually abused Leland Palmer.
It turns out that much of the movie was filmed for a TV pilot that didn't get taken on by ABC, so Lynch added more scenes for a feature length movie (bit of a red flag if you ask me), but all the pretentious people come out of the woodwork claiming how it's an insightful, profound masterpiece of some sort, when it clearly isn't.
