Near the beginning of Chapter 3 there is an X-ray puzzle you need to solve. When you awaken in the hospital you will see an out of service elevator that requires two batteries to open. There is however a corridor which has an examination room with an X-ray machine inside.

This section of IGN's Little Nightmares 2 wiki guide will walk you through the X-ray puzzle. This is one of the first puzzles you'll encounter after stepping foot in the Hospital.
Go past the elevator into the X-ray room. You’ll notice by the scans on the wall that a key is hidden in a stuffed animal, which is in the next room.
Unfortunately, even though it’s hidden in the stuffed bear in the photo, it’s actually in a different stuffed animal now. You’ll have to check out each one in the X-ray, but you’ll eventually find it in the
Once you have the one with the key, grab it and head into the elevator right outside this room. This will take you down to an incinerator. So exit the elevator and head right. Push the stuffed animal in the hole, then pull the lever.
Once it’s done, hop back in and grab the key. Then, take the elevator back up and head up to the room with the locked door on the second level to the right of the staircase. Open it up and head inside.

You will find the key in one of the soft toys. You can use an x-ray machine to search for it. The key is in the teddy bear lying by the cabinet on the right side of the room.
I have noticed that depending on which of the 3 stuffed animals mono picks up will determine which of the 3 toys Six will pick up. Six well then carry to the X-ray machine with Mono and reveal hidden inside a mirror a ax head or what appears to be a baseball depending on the toy she's holding.
