I can't answer all your questions but I'll make a start: Spawning sphere is between 24 and 44 blocks from the player's eyes. Beyond 44 blocks mobs instantly despawn. Beyond 32 blocks they have a small ‰age chance to despawn after 30 seconds. There are separate caps for surface and cave spawns, so getting both is good.
