In regards to 99% of the game, faction choice does not matter right now. There are voice lines in the game you can access by double-tapping Y while in raid (these can be hotkeyed). BEAR speaks russian, USEC speaks english.
It's more down to preference than anything else, but here's the differences: BEAR gets Russian equipment at start (AK-74, PP-19, Grach, etc), they speak either Russian or english with a Russian accent and they have a selection of faces and clothing that USEC cannot use.
