You'd have to build a pretty damn thick cavity wall if you wanted to use rough stone , or 6" furring that would be adequate to support the second floor all on it's own. I've designed a house with incredibly thick rammed earth "veneer" and interior structure, so things like that are possible, but the client is wasting a ton of money for the look.
In NYC many nicer older buildings have been completely gutted and equivently a new building was built on the inside with the outside staying the same. Chase did this to their headquarters 20 years ago. Though now the building is still functionally obsolete still and they torn it down and building a new building.
Transforming an "old" building into a "historic" building is a chancy process. There was a preservation movement before the restoration of the White House and before the passage of the 1966 act, but it was scattered and lacked the strength of unity and of a national platform for advocacy.
