Saying "Mr Smith" is more formal and polite than saying "Alan" which is more casual and personal. In Japan they have several other levels, way more than two but most cultures have at least two. If you don't think we need to have gender differentiated honorifics, that's fine, we can just call everyone Mr and make it gender neutral or something.
