If I remember correctly, on the BBC World Book Club podcast Lionel Shriver discusses how everything we know about Kevin is told to us from the mother's point of view. We have no way of knowing how truthful she is being to us or to herself and we are encouraged to question her veracity.
It broke my heart when they would show flashbacks of her life before she had that kid, where she was travelling and happy, loving her newly married life, and then she ends up becoming nothing other than a prisoner in a life she didn't really want to begin with, and that life turns into a nightmare.
