An employer’s filing frequency for state income tax withholding is determined each calendar year by the combined amount of state and school district taxes that were withheld or required to be withheld during the 12-month period ending June 30 of the preceding calendar year (i.e., total state and school district income tax withheld for 7/1/19 – 6/30/20 will determine the 2021 filing frequency).

Ohio Withholding Tax Returns: Effective Jan. 1, 2015, in accor-dance with Ohio Administrative Code rule 5703-7-19, employers are required to file state and school district income tax withholding returns and make payment of the withheld taxes through the OBG. Employers subject to withholding must make payments in the amounts required to be withheld.
The Forms an Employer Must File: • IT 501 Payment of Income Tax Withheld o To report and pay the Ohio income tax withheld for the period. o Employers are not required to file if no Ohio Income tax was withheld or required to be withheld for the period. o The filing of all IT 501’s does not complete your filing requirement for the tax year.
Visit the Ohio Department of Taxation website and navigate to the Tax Forms portal. Type IT 4 in the search bar titled Form Title or Number. Ensure you select the form called IT 4, Employee’s Withholding Exemption Certificate. Select your preferred version of the form: the standard PDF file or the fill-in version.
