Health assessments are usually structured screening and assessment tools used in primary care practices to help the health care team and patient develop a plan of care. Health assessment information can also help the health care team understand the needs of its overall population of patients.

The complete subjective health assessment is commonly referred to as a health history. It provides an overview of the client’s current and past health and illness state. You conduct it by interviewing the client as illustrated in Figure 1.1, asking them questions, and listening to their narrative. Figure 1.1: Nurse interviewing the client.

What is a health assessment? A health assessment is a set of questions, answered by patients, that asks about personal behaviors, risks, life-changing events, health goals and priorities, and overall health.
There are many tools to help you further explore a client’s symptoms or signs. A common one follows the mnemonic PQRSTU as illustrated in Figure 2.2, which offers a systematic approach to asking assessment questions without leaving out any details. Figure 2.2: The PQRSTU assessment mnemonic.

A brief questionnaire such as the free, online Patient Health Questionnaire (PHQ-9) aids diagnosis and assesses severity of the depression. As a screening tool, the PHQ-9 can assist in diagnosis, and also serve as a symptom severity tracker to help assess the effectiveness of the treatment plan.
In this chapter, we refer to four types including: primary survey; focused assessment; head-to-toe (abbreviated version); and complete health assessment. As per Table 5.1 , the type of health assessment to be performed is determined based on the context of the situation.
