It is the first installment of the now long running Madden NFL series, and was released for the Commodore 64, Commodore 128, Apple II, and MS-DOS. This was the first and only Madden game to be released on the Commodore and the Apple II. The Apple II version came out on June 1, 1988 and the MS-DOS, Commodore 62, and Commodore 128 versions came out in 1989.

    The namesake coach John Madden is on the front cover. Unlike future games in the series, it does not have an official NFL license and was not released on home consoles.

    The game is a computer simulation of the real life American football game. A football game starts out with a coin toss determining what team gets to choose to kick or receive, or which side to defend on. After the first team selects, the other team must choose an option from the other category, for example, if a team chooses to receive the other team's kick, the losing team must choose which side they are to defend.

    The team who is not chosen to receive then kicks a football to the other team, with other team players attempting to catch the ball as far away from their end zone as possible. Once caught, the player runs as close to the end zone of the opposing team before they get tackled and fall to the ground.

    The main game loop consists of attempting to reach the opponent's end zone in order to score a touchdown to gain points. The offense team, or the team who currently has possession of the ball, can choose to either start a running play and attempt to rush to the other side of the field, or start a pass play, where a quarterback throws the ball to another offense team player, and if the ball is caught the play then continues similarly to a running play. After the play finishes, the next play starts the ball where the ball carrier from the last play was tackled, unless the last play was a passing play and no player successfully caught the ball in the stadium boundary.

    The team without the ball is labeled the defensive team, and the defense team must attempt to gain possession of the ball in order to have a chance to score. The offense team has four plays, to attempt to advance a required distance, referred to as downs in this context. If the team does not advance the required distance on the stadium, the possession of the ball switches to the other team and can start advancing from where the last play ended. The defense team can also gain possession of the ball by performing an interception, or catching the ball thrown by the other team, or by a defensive team picking up the ball if it is dropped irregularly in a fumble.

    On the fourth down, the offensive team usually punts the ball to the other side of the field to increase the amount of distance the opposing team must travel to get a touchdown. If the offense team is close enough to the defense team's end zone, they may attempt a field goal challenge in an attempt to get some points instead of failing fourth down and losing possession of the ball. If the ball carrier gets tackled in the offensive end zone, a safety may occur, which causes the defense team to earn points.

    The game has customizable features like weather conditions, quarter lengths, injuries, fatigue, and penalties. There is only one mode, Play Now. You can only play one game at a time since there is no season mode. You can play the game by yourself or with a friend in multiplayer.

    There are no NFL or NFLPA licenses, so there are no real players, stadiums, or teams. Instead, teams are made up. There is also an option to create your own team.
